We will mail a hard copy of the directory to you within three (3) business days of your request. You can request a provider directory in an alternate accessible format if needed.

Gold Kidney Health Plan, Inc., is an HMO-POS and HMO-POS C-SNP with a Medicare contract. Enrollment in Gold Kidney Health Plan depends on contract renewal.
