If you identify an error, need help finding a network provider, or want to request a hard copy of the Gold Kidney Health Plan provider directory, please call Member Services at (844) 294-6535 (TTY 711). We will mail a hard copy of the directory to you within three (3) business days of your request. You can request a provider directory in an alternate accessible format if needed.

Gold Kidney Health Plan, Inc., is an HMO-POS and HMO-POS C-SNP with a Medicare contract. Enrollment in Gold Kidney Health Plan depends on contract renewal.
